Stay Tuned – Exciting Collaborative Practice Services Coming Soon!
(This section is reserved for a future service offering currently in development. Custom Care Pharmacy Services is actively working on new “Collaborative Practice” initiatives that will further integrate our pharmacists into the healthcare team, possibly including services like Chronic Disease Management, Pharmacogenetic Testing, or Advanced Therapeutic Consults in collaboration with physicians and nurse practitioners. Details will be announced once available.)
For now, we want our clients to know that we’re always innovating. The Collaborative Practice area is where we plan to introduce next-level pharmacy services under formal agreements with prescribing providers – all aimed at improving resident outcomes even more. This could involve our pharmacists taking a more direct role in managing certain conditions (under protocol) or offering clinical clinics within facilities (for example, a pharmacist-led anticoagulation monitoring service).
What you can expect:
- The same commitment to personalized, high-quality care, now expanded into new clinical domains.
- Clear communication about any new program and how it benefits your community.
- Optional participation – we’ll tailor collaborative practice services to interested partners and ensure they align with your needs and preferences.
We are excited about the possibilities and are working diligently behind the scenes to bring these innovative services to life. Please check back later for updates on our Collaborative Practice offerings. In the meantime, if you have ideas or needs that you think could be met by an expanded pharmacist role, we welcome your input! Custom Care has grown by listening to our partners, and this future service is no exception. Thank you for your interest. More information will be added here as soon as it’s ready.
