Custom Care Pharmacy Services is actively working on new “Collaborative Practice” initiatives that will further integrate our pharmacists into the healthcare team, possibly including services like Chronic Disease Management, Pharmacogenetic Testing, or Advanced Therapeutic Consults in collaboration with physicians and nurse practitioners. Details will be announced once available.)

For now, we want our clients to know that we’re always innovating. The Collaborative Practice area is where we plan to introduce next-level pharmacy services under formal agreements with prescribing providers – all aimed at improving resident outcomes even more. This could involve our pharmacists taking a more direct role in managing certain conditions (under protocol) or offering clinical clinics within facilities (for example, a pharmacist-led anticoagulation monitoring service).
