From Personal Frustration to Personalized Care – The Custom Care Story

Our story begins over a decade ago with a simple observation: the conventional pharmacy model was not meeting the needs of long-term care communities. Adam Rosinski, our founder, experienced this firsthand working as a pharmacist for large chain pharmacies and also serving as an administrator for personal care homes. He saw facilities struggle with inflexible services, frequent errors, impersonal support, and a general lack of understanding from their pharmacy providers. As he says, “I was tired of the one-size-fits-all service . Our residents and staff deserved better.”

2012 – The Founding: Determined to create a better solution, Adam established Custom Care Pharmacy in 2012 . Starting as a small independent pharmacy in Milton, PA, he focused on building genuine customer relationships and offering specialized services that big-box stores wouldn’t – like free local delivery, medication compounding, and personalized medication packaging . The pharmacy’s name “Custom Care” reflected a promise: tailor the care to each individual’s needs. Early on, Adam introduced adherence packaging for seniors living at home and offered one-on-one consultations, differentiating his business with a personal touch.

Expansion into Long-Term Care: As word spread about the quality service, local personal care homes and assisted living facilities took notice. They began asking Custom Care to handle their residents’ medications. Adam eagerly took on this role, seeing it as an opportunity to apply his administrative insights. By 2015, Custom Care Pharmacy was serving several facilities in Central PA. Adam hired pharmacy technicians and drivers specifically for LTC services, ensuring timely cycle fills and deliveries. We invested in our first strip packaging machine around this time to accommodate an ALF’s request for multi-dose packs – an investment that paid off as more communities loved the convenience.

Built by an Administrator – 2017 Milestone: In 2017, Adam himself became a licensed Personal Care Home Administrator, taking on operational responsibility for a local personal care residence. This was a pivotal moment in our origin story. Now at the helm of both a care facility and a pharmacy, Adam gained an even deeper appreciation of the day-to-day challenges – and directly infused those learnings into Custom Care’s services. For example, he understood how disruptive late medication deliveries can be to a caregiver’s evening med pass, so he adjusted our delivery routes and cut-off times to avoid that scenario. He experienced the pains of medication cost on a facility’s budget, leading him to develop our cost containment consulting. Essentially, the pharmacy evolved to solve the exact problems he faced as an operator – making Custom Care a pharmacy truly designed by a long-term care operator for long-term care operators. This remains one of our strongest differentiators today.

2018 – Formalizing LTC Services Division: As our client base grew, we officially formed the “Custom Care LTC” division (hence our new website, customcareltc.com). We hired additional consultant pharmacists and nurse educators to expand our support services. We also adopted more robust pharmacy software to integrate with facility eMARs and rolled out our online portal. By this time, we were serving not just elderly communities but also IDD/behavioral group homes. We fine-tuned offerings like specialized packaging for group homes (color-coded MAR pouches) and became known for our responsiveness and willingness to customize. A story from around this time: A behavioral health provider had residents frequently transitioning in and out for short-term treatments. They struggled with their previous pharmacy refusing to take back unused meds or adapt to quick changes. We stepped in and created a flexible return-credit policy and a rapid admission kit, winning their trust and demonstrating our nimble approach. Many such experiences built our reputation across Pennsylvania.

Joining Forces & Technology Upgrades: In 2020, we partnered with a leading pharmacy technology firm to upgrade our automation. We installed a new state-of-the-art robotic dispensing system (nicknamed “ROSA”) that significantly increased our capacity while maintaining accuracy. This enabled us to scale up to serve larger communities and skilled facilities as well. We also enhanced our communication systems – introducing a 24/7 hotline for urgent needs and integrating secure texting for quick updates with nursing staff. These enhancements were timely, as the COVID-19 pandemic hit and challenged everyone. During the pandemic, Custom Care’s commitment shone through: we managed to keep all facilities supplied (even sourcing scarce medications), assisted with vaccine clinics by providing pharmacist immunizers, and were available around the clock as a steady support. It was an intense period that reinforced our mission’s importance.

Present Day: Today, Custom Care Pharmacy Services proudly serves a wide array of long-term care providers throughout Pennsylvania – from small 8-bed personal care homes to 200-resident assisted living communities, from group homes for individuals with intellectual disabilities to specialized memory care units. We’ve grown in size, but remain true to our local roots and personal touch. Adam still personally calls new clients to welcome them and periodically checks in at facilities unannounced just to see how things are going (often with donuts for the staff!). Our growth has been organic, largely through word-of-mouth referrals, which we consider the highest compliment.

Our journey from a single-store pharmacy to a specialized LTC provider has been guided by one question: “How can we help our care partners do their jobs better?” Every service we developed, every innovation we adopted, was in answer to that question. Our history is one of listening and evolving – and we continue to write new chapters as we find better ways to serve.
